Notice on Health Declaration Code Application Requirement for Passengers Traveling to China


1. At present, the global COVID-19 and Omikeron variant pandemic is severe, due to the high risk of cross-infection during international travel, passengers are kindly advised to follow the principle of no travel for non-essential or non-urgent reasons.


2. Passengers who travel to China for essential reasons are required to complete the COVID-19 vaccination in advance, and arrange your trip to China 14 days after the final injection.


3. Please self-quarantine and monitor health condition 14 days before going to China. For suspected COVID-19 symptoms such as fever and cough, please cancel your trip immediately and report to the Embassy and receive treatment. Please take required tests and apply Health Declaration Certificate (HDC) before departure and take necessary protection measures during the journey to minimize the risk of infection.


4. Test items and requirements


(1) All relevant tests shall be completed in one of the Greek clinics designated by Chinese Embassy within 48 hours before boarding ( For example: The test sampling is on January 3rd, and the validity period is until January 5th ).  Please provide the designated clinics with your passport and 2-inch photograph and agree to let them send your test reports to Chinese Embassy at hsm67b@gmail.com.


(2) For passengers who vaccinated with inactivated COVID-19 vaccines (China's SINOPHARM and SINOVAC vaccines,etc.), please conduct 2 PCR tests at two different designated clinics at intervals 24 hours each within 48 hours before boarding. For example, people who takes the first test at LIFECHECK clinic at 9 am on January 3rd should take the second test at MEDICITY clinic on January 4th after 9 am. In case of clinic closing on holiday, the first PCR test can be performed one day in advance, that is, on January 2nd. If both two PCR test results are negative, please submit your test results, Vaccination Certificate and "Letter of Commitment on COVID-19 Vaccination" (see the attachment, travelers must fill out the attachment truthfully by themselves and sign with handwritten signature).


Vaccinated travelers whose PCR test result shows positive should cancel travel plans and receive treatment.


(3) For Passengers who vaccinated with non-inactivated COVID-19 vaccines (Pfizer, Moderna, J&J and Astrazeneca vaccines,etc.), please conduct PCR test and anti-N protein test. If both test results are negative, please submit your test results,   vaccination certificate and the “Letter of Commitment on COVID-19 Vaccination” . Vaccinated travelers whose PCR or anti-N protein test result shows positive should cancel travel plans and receive treatment. After the test results turn to negative, travelers should follow the procedures of those who have infection record.


(4) For Passengers who have not been vaccinated with the COVID-19 vaccines should conduct PCR test and IgM antibody test. If the test results are both negative, please upload the test results and apply for health code. Unvaccinated travelers whose PCR or IgM antibody test result shows positive should cancel travel plans and receive treatment. After the test results turn to negative, travelers should follow the procedures of those who have infection record. 


(5) For Passengers who have been vaccinated with inactivated and non-inactivated vaccines, please take tests and apply for health code according to the procedures for people who take inactivated vaccines.


5. Health Declaration Certificate Application Process


(1) Foreign passengers should take relevant tests in clinics which designated by the Chinese Embassy and apply code by logging on the website of https://hrhk.cs.mfa.gov.cn/h5/ via computer or smart phone, or by scanning the QR Code below. Passengers need to register, fill in the information, declare the health status and upload the test certificates, passport, Chinese Visa, Vaccination Certificate and "Letter of Commitment on COVID-19 Vaccination" online, which will then be examined and verified by the Embassy. Upon approval, applicants will receive the Electronic Health Declaration Certificate, which will be displayed in QR Code with the “HDC” mark. Foreign passengers must present the electronic or printed QR code with the “HDC” mark, and follow the procedures for inspection by relevant airlines during boarding. 


(2) Air China requires passengers flying from Athens to Beijing show documents for check-in as following: 1. The "HDC" mark (for foreign passengers) issued by the Chinese Embassy. 2. A screen shot of receipt after signing the Air China Passenger Notification Letter and Personal Commitment documents. 3. Paper copies of all documents used when applying for the Health Declaration Certificate, including all relevant test reports. For transit passengers, test reports issued in Greece (transit country), as well as the country of departure are both required.


6. Passengers who are diagnosed with Covid-19 by medical institution, please follow the guidance of Greek government to stay quarantine and take medical treatment. After recovery from Covid-19, passengers who still have to travel to China for essential reasons are required to take the following steps:


(1) Get a CT/X-ray scan on lungs at one local qualified hospital and take Covid PCR test twice at designated clinics by Chinese Embassy in Greece (minimum 24 hours between 2 PCR tests).


(2) Send the CT/X-ray scan report and the two negative PCR test reports to hsm67b@gmail.com by email for checking.


(3) After receiving approval email from the Embassy, please stay self-quarantined for 14 days. Take required tests and apply for HDC only if no symptoms related to Covid-19 shows during the quarantine.


For those who have unprotected contacts with confirmed Covid-19 patients or asymptomatic coronavirus carriers, please stay self quarantine for at least 14 days after the unprotected contact and take PCR tests on days 1, 4 and 7 during the monitoring period. If the results are all negative, please apply for HDC after completing relevant tests.  


7. Any falsification or alteration of the test certificates may cause the application or boarding to be rejected and even bring about legal liabilities on the part of the applicants.
